Tax planning for physicians: high income deserves a real plan.

Physicians often reach the highest tax brackets early and stay there. Owner-physicians, employed doctors with side income and practice groups each have different planning opportunities. We build a plan for each.

What we handle

Where physicians save

For practice owners, the starting points are entity structure, a defensible owner salary, the state pass-through entity tax election and retirement plans. Health is a specified service business, so the section 199A deduction phases out at higher incomes. A cash balance plan paired with a 401(k) often produces the largest single deduction.

Employed physicians with locum, consulting or expert witness income can use a separate entity and solo 401(k). Equipment, medical office build-outs and practice-owned real estate add depreciation planning, and backdoor Roth contributions and charitable strategies help with long-term savings.

Questions

Frequently asked

If your question isn't here, call (703) 880-8412 and you'll reach a person.

Should my practice be an S corporation?

Often yes for owner-physicians with profits well above a reasonable salary, but state rules for professional entities, partner arrangements and retirement plans affect the answer.

Can I get the QBI deduction as a physician?

Only below the income phase-out range, because medicine is a specified service business.

How should I handle locum or consulting income?

It is self-employment income. A separate entity, solo 401(k) and careful expense tracking can reduce tax on it.

Is a cash balance plan right for my practice?

It usually fits practices with stable profits and owners age 45 or older. We model contributions and staff costs first.
